The Frailty Team

Who are we?

The Frailty Team are a multidisciplinary team (MDT) of frailty nurses, advanced clinical practitioners, medical doctors, nurses, healthcare assistants, physiotherapists, occupational therapists, therapy assistant practitioners and administrators.

Where are we?

We work across Accident & Emergency (A&E), the Frailty Assessment Unit (FAU) and Rockwood Short Stay Unit.

What we do

All frailty practitioners are trusted assessors which means that whilst you are on any of these wards/units you may be seen by any member of the team. If you need to be seen specifically by a particular part of the team, then this will be arranged. All members of the MDT work following a particular format known as a Comprehensive Geriatric Assessment (CGA) so you can rest assured that your care will always be carried out in a very holistic way.

During your time with us we will liaise with those people who are important to you to help us gain a picture of the things that are important to you and of the things that have been causing you difficulty leading to your admission to hospital.

Our aim is to get you home as soon as you are ready, which means that patients usually stay as an inpatient for a very short period of time before either being discharged home (sometimes with support e.g. an individualised package of care, where necessary) or being transferred to another ward if further therapy or medical intervention is required
