At The Hillingdon Hospitals NHS Foundation Trust, we believe every person affected by cancer deserves care that is tailored to their individual needs. Our Personalised Cancer Care programme ensures you receive the right support, at the right time, throughout your cancer journey.
What is Personalised Cancer Care?
Personalised cancer care means focusing on what matters most to you. It is part of the NHS Long Term Plan and supported by Macmillan Cancer Support. Our approach includes:
- Holistic Needs Assessment (HNA) and Personalised Care and Support Plan Offered at diagnosis and key transition points to understand your physical, emotional, and practical needs. Co-created with you to set goals and plan your care.
- End-of-Treatment Summary - Shared with you and your GP to ensure continuity of care.
- Cancer Care Reviews (CCR) - is a conversation between you and a health or Social Care professional. You may be invited automatically for a review, or you can request one by contacting your GP practice.
- Health & Wellbeing Support - Access to information, workshops, and services to help you live well. · Quality of Life Survey and National Cancer Patient Survey Helping us monitor and improve your experience.
Personalised cancer care is about partnership - between you, your family, and your healthcare team. We are here to support you every step of the way.

Holistic Needs Assessment (HNA)
Your Holistic Needs Assessment (HNA)
The HNA is a simple questionnaire designed to help us understand what matters most to you as you live with cancer. It's your chance to tell us about any worries or needs you may have, so we can support you in the best possible way.
Most people complete the HNA electronically, but if you'd prefer a paper copy, just let one of your healthcare professionals know and they'll be happy to provide one.
By completing the HNA, you help us identify the areas where you need the most support-whether that's physical, emotional, practical, spiritual or social. After you've filled it in, a member of your care team will sit down with you to talk through your answers and work with you to create a personalised care and support plan that's right for you.
You'll be offered an HNA at key points during your treatment journey, so we can make sure your care reflects your individual needs. Please do complete it when offered-it really helps us to give you the care and support you deserve.
Remember, you're not alone on this journey. We're here to listen, support, and walk alongside you every step of the way.

Care and support planning - Your Personalised Care and Support Plan
This plan is created as an outcome of your Holistic Needs Assessment. It's a record of the conversation you've had with your healthcare professional, capturing what matters most to you and the support you need. You can keep this plan and share it with your GP to make sure everyone involved in your care is on the same page and working together for your wellbeing.
It's your plan, designed around your life and your goals and what matters to you.
End of Treatment Summary
Your Treatment Summary
After you finish a phase of your treatment (this may not be the end of all your treatments), a member of your care team will prepare a Treatment Summary for you.
This summary will include:
- Details of the treatments or surgery you've had
- Possible outcomes or side effects of those treatments
- Signs and symptoms to look out for that might indicate a recurrence
Your Treatment Summary will be shared with you and your GP. It gives your GP up-to-date information and lets them know if there are any actions they need to take. It also helps them understand the care you've received so they can support you in the best possible way.
This summary is an important part of your ongoing care and helps everyone involved work together for your wellbeing.
Cancer Care Review
Cancer Care Review (CCR) is a conversation between a patient and a health or Social Care professional. You may get invited automatically for a cancer care review or if you would like one, please contact your GP Practice. The review can cover physical symptoms, emotional wellbeing, medications, and any ongoing concerns. The review helps you to share what matter to you, get clear information and feel supported in managing your health.
