Primary care services and neighbourhood-based models of care
To ensure more equitable and accessible healthcare for the 319,000 residents of Hillingdon, there is a compelling case for elevating the role of primary care services and neighbourhood-based models of care.
At our Trust, we are currently reviewing the efficiency and equity of all our services through the lens of the health inequities present in our borough and also working with North West London ICB on ensuring our residents are aware on what is available on their doorstep. Our focus remains clear: to ensure those who are most unwell receive the care they need first.
The Urgent Care Nurse Practitioner Service at Mount Vernon Hospital will be reconfigured into the Urgent Treatment Centre at Hillingdon Hospital.
The last patients will be seen at the Urgent Care Nurse Practitioner Service on Friday 26 September. Patients should not attend after 4pm. You can find out more about the service here.
This will help us to strengthen and focus our resources at Hillingdon Hospital's Urgent Treatment Centre. By bringing services together, we are building a more resilient model of care that will improve patient experience and ensure we can provide the right care in the right place.
This change is an important step in strengthening our urgent care services and ensuring we continue to deliver high quality, safe and responsive care for our communities.
Aligned with the three key shifts of the NHS 10 year plan-boosting out-of-hospital care, reinforcing personalised care, and reducing health inequalities-this transformation would reposition primary care as the cornerstone of a more integrated, preventative, and community-focused system.
By strengthening neighbourhood hubs, services can be delivered closer to where people live, making care more responsive to local needs and reducing reliance on hospital-based interventions.
This approach not only improves access and continuity but also addresses longstanding disparities in health outcomes across the borough's diverse communities, ultimately creating a more sustainable and person-centred model of care.
