Primary care services and neighbourhood-based models of care

To ensure more equitable and accessible healthcare for the 319,000 residents of Hillingdon, there is a compelling case for elevating the role of primary care services and neighbourhood-based models of care.  

At our Trust, we are currently reviewing the efficiency and equity of all our services through the lens of the health inequities present in our borough and also working with North West London ICB on ensuring our residents are aware on what is available on their doorstep. Our focus remains clear: to ensure those who are most unwell receive the care they need first.

We are proposing the consolidation of minor injuries services into a single, clinically robust and financially sustainable model. 

This proposal involves bringing together the standalone Urgent Care Nurse Practitioner Service at Mount Vernon Hospital with the Urgent Treatment Centre at Hillingdon Hospital with the aim of optimising resources and enhancing access, safety, and equity of care across the borough.

Aligned with the three key shifts of the NHS 10 year plan—boosting out-of-hospital care, reinforcing personalised care, and reducing health inequalities—this transformation would reposition primary care as the cornerstone of a more integrated, preventative, and community-focused system.  

By strengthening neighbourhood hubs, services can be delivered closer to where people live, making care more responsive to local needs and reducing reliance on hospital-based interventions. 

This approach not only improves access and continuity but also addresses longstanding disparities in health outcomes across the borough’s diverse communities, ultimately creating a more sustainable and person-centred model of care.
