Care Information Exchange for children and young people

The Care Information Exchange is now available for children and young people under 18, giving you access to information about your hospital care such as appointments, clinic letters, test results and treatment plans.

Most patients aged 13 to 17 years can sign up for themselves. A parent or guardian will usually sign up for children under 13.

Please note, that the Trust has a duty to do a safeguarding check for every request to get access to a child's record. This is done for everyone. If clarification is needed, you may be asked to speak to your clinical team before access is given.

These are the steps to register for your account:

1. Fill in the registration form

This can be done on paper in clinic or with our secure online form. You will have to book a time slot for a video call to present an acceptable form of ID using the link at the end of the registration form.

  • Patients aged 13-17: passport, provisional drivers licence, or a transport pass with name and photo
  • Parent or guardian: driving licence, ID card, or passport and a copy or photo of the young person’s birth certificate.

If you prefer to present ID in person, you can bring this with you to your next appointment and ask a member of staff to check it.

2. Activate your account and start using it

The helpdesk team will now contact you to help you activate your account.

FAQs

What if I don’t want to sign up. Will it affect my care?

No, you will continue to receive information in the way you do now such as letters by post and text reminders. The Care Information is simply a way to see all that information all together on a secure website.

Can I use the NHS login to sign up?

Under 16s cannot use the NHS login to sign up for the Care Information Exchange. Healthcare professionals have additional responsibilities for keeping children and young people safe. This means a different sign up process to the one for those aged 16 and over. 

Can I be prevented from accessing my child’s record?

The Trust has a duty to make sure it’s safe for every parent or legal guardian to get access to their child's record. This is done for everyone. If clarification is needed, you may be asked to speak to your clinical team before access is given.

Can I have access to my own record as an Adult?

Yes, you can gain access to your own record as well as access to your child’s record. You can view your health record.

Are the Care Information Exchange and Patients Know Best the same thing?

Yes, the Care Information Exchange and Patients Know Best are the same thing. The Care Information Exchange is powered by Patients Know Best.

Can anybody else sign up to view my child’s information?

No, only the clinical team and parents or legal guardians can access the record.
