Going live with digital consent

We will always ask for your consent — recording that you have given permission for a test, examination, treatment, or operation. Previously, this may have been done on paper, but you can now complete this process online, using a website called Concentric.

By using Concentric, you can take as long as you need to read all the information about your test, treatment, or procedure before you give your consent, completing the form at home or at the hospital. Concentric is used by all the acute trusts in north west London.  

If you can’t complete the consent form online, don’t worry. We will print a paper copy for you to sign, which will be scanned into your patient record later. 

Please ask a member of staff about our move to digital consent.
